1) Long-term care to acute settings transfer - this is the case where a resident of a Skilled Nursing Facility or another Long Term Care facility needs to receive care in a hospital. There are at least too possible sub-cases, one where the patient returns to the same LTC facility, and the other where the patient goes to another facility, or is discharged to home.

2) Long-term care to another Long Term Care facility - this is a case where the patient needs to be transferred to another LTC facility.

3) E-consult - this is a use case where there is a formal request for consultation to a specialist in a case where the specialist does not need to actually see the patient. This is of rising importance in rural areas, where the referral initiator is local to the patient, and the consulting specialist is remote, but they still can offer valuable input to the care of the patient. This has the support of payers in several areas, where such consults are paid.

4) Community services referrals - this is the case where referrals are made to services to improve the social determinants of health (SDH) for the patient. These services are not necessarily medical, and therefore the usual medical information about the patient is not needed to be shared as part of the referral process.
