Utah Furs/U2F2 COVID-19 Self-Reporting Form

In the event a community member tests positive for COVID-19 during or after any United Utah Furry Fandom (U2F2) organized meets, the U2F2 Leadership Team hopes to help other community members determine if they might have had close contact with an exposed individual.

Your identity will be known only to the admins and you will remain completely anonymous to the rest of the community. The only information we will share to the community as a result of a self-report is where and when a potential exposure occurred.

Please report to the appropriate UT contact tracing app for contact tracing purposes!

Regardless of whether your test was self-administered or provider-administered, you can report it using UT app. Please download it here, even if you didn’t have it during the event:

This is an extra step, but we REALLY, really encourage it so that your results can be shared anonymously with people you came into contact with at-event, if they have the app downloaded as well. Thank you for being a public hero!
