BY BOOKING A SEAT FOR OUR IN-PERSON CHURCH SERVICE YOU UNDERSTAND AND AGREE TO THE FOLLOWING:

I am currently NOT experiencing any of the following symptoms: fever, cough, sore throat, shortness of breath, difficulty breathing, body aches, loss of smell or taste, nausea, vomiting, diarrhoea, fatigue, weakness.

If I experience any of the above symptoms on the day I have booked in for, I will not attend the service.

I agree to wear a face mask at all times covering both my nose and mouth.

I agree to have my hands sanitized upon arrival.

I agree to adhere to the social distancing practices stipulated by the Health Department of South Africa and the church.

I agree to the church recording my personal contact details for the purposes of ‘contact tracing’ should the need arise.

I agree that if I have been exposed to Covid-19 I will not attend services within 14 days of said exposure.

I agree that if I have tested positive for COVID -19, I will immediately inform any church leader if I have been at the church in the 14 days prior to being tested positive.

INDEMNITY: I understand that I am personally taking the risk of potentially being exposed to COVID-19 by attending the service. I will not hold Tokai Community Church or REACH SA liable for any infection or illness that occurs as a result of my attendance. *
