Thank you for participating in the CHIP (Community Health Improvement Plan) Check Tool.
The purpose of the CHIP Check Tool is to assess how the operations and policies of an entity aligns with the CHIP Action Agenda. The CHIP Action Agenda is a set of best practice principles for health equity that was informed by nearly 100 participating members of the Greater Worcester Community.
The CHIP Check Tool is not an evaluation or audit of the respondent. Instead, it is an invitation to engage directly with the Coalition and the Community Health Improvement Plan.
What is the CHIP Action Agenda? We are working with other local community groups to address health issues persistent on the community level. The following six guiding principles will be planned and implemented to address upstream system issues with the goal of creating more equitable avenues to resources and care, and as a result, greater equitable health outcomes across race, ethnicity, gender, and socioeconomic status in the Greater Worcester Region.
Target Respondents: Entities that help the community improve public health and/or provide social needs essential for good health (example: housing, public benefits, insurance coverage, access to transportation, etc.). Please respond as a representative of the entity, not a private individual.
