Parent or Guardian permission and release *
Parents/Guardians: The Upstate New York Math Team requires that you agree to the following for your child to participate in any activity: As the parent or guardian, I hereby authorize my child, if invited, to participate in ARML on June 2–3, 2023 with the Upstate New York Math Team. I understand that this is an in-person event, where team members will gather at Penn State in State College, PA, and compete in ARML together. I hereby release The Upstate New York Math Team, its directors, officers, employees, students, agents, representatives or affiliates, from any and all liability as a result of property damage, bodily injury, or personal injury to the above named minor participant resulting from the above named minor’s participation, if not caused by the sole negligence of The Upstate New York Math Team. I further understand that all The Upstate New York Math Team employees including instructors, staff and volunteers involved in said programming will exercise reasonable care and act responsibly while conducting activities of said programming.
