Due to the outbreak of the Coronavirus (COVID-19), your artist is taking extra precautions with the care of every guest to include a health history review and enhanced sanitation procedures in accordance with the Local, State, and CDC Guidelines.

Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:*

✼ Fever or Chills
✼ Cough
✼ Shortness of breath
✼ Difficulty breathing
✼ Fatigue
✼ Muscle or body aches
✼ Headaches
✼ New loss of taste/smell
✼ Sore throat
✼ Congestion or runny nose
✼ Nauseas or vomiting
✼ Diarrhea

*This list does not include all possible symptoms. The CDC will continue to update this list as we learn more about COVID-19.

I understand the above symptoms and confirm that I, as well as all household members, do NOT currently have, nor have experienced the symptoms listed above WITHIN THE LAST 14 DAYS.

I confirm that I, as well as all household members, have NOT been diagnosed with COVID-19 WITHIN THE PAST 14 DAYS.

I confirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 14 DAYS.

I have not, nor has anyone in my household, visited any area within/outside the United States that was reported to be highly affected by COVID-19 in the past 30 days.

I am fully and personally responsible for my own safety and actions while/during my participation and I recognize that I may be at risk of contracting COVID-19. I waive all liability of my stylist, including all performing staff at Clawdia's Beauty Salon during my visit.
