2023-2024 Nazareth Area High School Parental Consent Form for Student Release: Study Hall and Eagle Block/End of Day Homeroom Application (11th & 12th graders ONLY) 
In order for students to be released during scheduled study halls and/or Eagle Block/end of day Homeroom , they must meet the 3 eligibility requirements below AND have parent/guardian permission. Parent/Guardian must check the appropriate box(es) and submit this form to grant permission.

New this year! To be eligible for Study Hall and Eagle Block Release, a student must meet ALL of the following criteria:

    • Is a current 11th or 12th student

    • Is not habitually truant during the current school year (18 or more unexcused blocks)

    • Passed 2 or more course credits the previous semester

***Students who do NOT meet all 3 criteria will NOT be granted release at this time. Juniors or seniors can re-apply for SH or EB release next semester if they do not meet the attendance and academic criteria at this time.


Parent/Guardian only has to submit the form one time per child per school year. Please make sure to read the Student Release Guidelines document that is linked below for all of the eligibility criteria, specific details, start dates, and requirements for each release type. This form must be completed separately for each high school student.
NOTE: "Academic Support" class is not considered a study hall and therefore is not eligible for release.

FOR STUDY HALL RELEASE:  I have read the 2023-2024 STUDY HALL Release Guidelines and verify my student meets the three eligibility criteria. I fully understand that beginning on 9/11/23 AND once my student's schedule is changed from "Study Hall" to "Study Hall Release" in PowerSchool,  my student will be expected to leave campus for study hall.

FOR EAGLE BLOCK/END OF DAY HOMEROOM RELEASE:  I have read the 2023-2024 Eagle Block/End of Day Homeroom Release Guidelines and verify my student meets the three eligibility criteria. I fully understand that beginning on 9/11/23,  my student will be permitted to not be on campus during Eagle Block/End of Day Homeroom as long as he/she is not tagged by a teacher to attend an Eagle Block activity. My student must also select "Eagle Block Release" in FlexTime Manager for each day he/she plans to participate in Eagle Block Release once Eagle Block activities begin.
