The total cost of the trip is $115 per student. A $25 non-refundable deposit is due at registration to ensure your students spot. The final balance is due February 1st. A late fee of $15 will be added to all payments after this date.

Consent to Attend ELEVATE High School Retreat
We must have a medical release form signed for every student attending this retreat.  Copies are available at the Student Welcome Desk or on our retreat website.
I give my student permission to attend ELEVATE High School Retreat 23.  By signing this form, I agree to hold harmless Sherwood Baptist Church and the adult sponsors from any and all liability, claims, or demands for personal injury, as well as damage and expenses of any nature that occur while my student is participating in this event.  I give permission for my student to receive medical attention if required. (Parents will be contacted in case of an emergency.)
