Collegiate Athletics Recognition Ceremony Interest Form
At the end of each academic year, the North Haven Athletic Department recognizes all NHHS Student-Athletes who have decided to continue their athletic endeavors at the collegiate level.  This ceremony will include those student-athletes who have already committed to a secondary institution by signing a National Letter of Intent (NLI) and those student-athletes who have recently committed to a secondary insititution (who may have not had an opportunity to sign a NLI). Please note:
- Although this event is not an official signing ceremony, student-athletes will have an opportunity to take pictures in front of the North Haven Athletics backdrop.
- This form verifies the intent of the student-athlete to compete athletically at the collegiate level.

This year's event will take place on Thursday, June 1, 11:00 AM in the North Haven High School Theater.

By committing to be a part of this recognition ceremony, the student-athlete & parent/guardian attests that the student-athlete has communicated with the coach of the aforementioned college/university and that the coach of the college/university expects the student-athlete to play the collegiate sport mentioned above in the next academic year.
PRESS RELEASE AUTHORIZATION:
I understand that the North Haven High School Athletic Department will recognize me/my son/my daughter at a Collegiate Athletic Recognition Ceremony. I hereby grant North Haven High School and the North Haven High School Athletic Department permission to include me/my son/my daughter in any form of a press release from this event, including pictures and other written and online communication.
