This form should be used by visiting teams prior to any game hosted by WSSL. Please submit on the day of the game at least 2 hours or more before the start of the game.

It is important to be honest in completing this questionnaire. It will assist us in trying to keep all of our families safe. IT IS ALSO IMPORTANT TO COMPLETE THIS QUESTIONNAIRE ON THE SAME DAY AS THE GAME (PLEASE DO NOT COMPLETE THE NIGHT BEFORE).
