This form is to be completed by any Lockport student going into grades 7-12 who is interested in trying out for cheerleading during the 2023-2024 school year.

The JV/Varsity commitment for athletes is 6 days per week, usually Monday-Saturday; please check below whether or not you understand and agree to this time commitment should you be chosen for one of the teams.  *
