When should you submit this form?

1. If you are unhappy with your order and want to return the product.
2. You can only return the product if it is in its original, unused condition. No returns are allowed if the product has been charged and used. If you receive defective item, please refer to another form. 

Eligibility:
1. You must request a return within 30 days of receiving your shipment at your address.
2. The product must be in perfect, untouched condition.
3. Please refer to our return policy for more information.

If you do not meet the criteria mentioned above and still return the product, you will not be eligible for a refund. Please note that all shipping fees for returns are the responsibility of the buyer.
