This form is required to be completed by all coaches, managers, treasurers and GMBA board members as a declaration of completion of OBA requirements for volunteering with GMBA,

All vounteers with GMBA (coaches, managers, treasurers) are required to complete:
1) Volunteer Screening Disclosure 
2) Baseball Ontario Safe Supervision
3) Safe Sport Training Module

**The easiest way to do this is to install the Baseball Ontario OnDeck  App to complete the volunteer screening.  You have options to register as a coach, parent or "other".  There are links to required modules within this screening as well.
