This summary is your chance to tell the selection
panel how you know the nominee, what makes them great, and provide examples of
how they demonstrate outstanding service to our community.
To make your nomination as strong as possible,
please consider the following questions:

the initiatives or projects of the nominee.
- Role(s): State the nominee’s role in each
initiative/project (e.g. volunteer, participant, leader, coach, organizer,
founder, etc.)
- Time commitment: Describe the nominee's time commitment
to each initiative/project and the number of years the nominee has been
involved.
- Results/ Impact: Provide specific examples of how the
nominee’s contributions have made a difference. What was accomplished? How is
the community better because of the involvement of the nominee? Would the
initiative/ project have taken place without them?
- Reach: How far does the impact go? Who has
benefitted from the nominee’s actions? How has the nominee inspired others?
- Other unique considerations: Is there anything
unique we should know about the nominee or challenges they’ve overcome?
