Dear Athlete, Parent, or Guardian:
An individual’s total education extends beyond the classroom. The purpose of the District 50 extra-curricular program is to provide each student with opportunities for emotional, cultural, mental, physical, and social growth by participating in student-selected activities. This growth can be accomplished through activities that support and advance the curriculum, promote responsibility and teamwork, develop a sense of ownership in the school, and reflect and enhance the goals of District 50. Such participation is a privilege that carries with it a responsibility to teammates, the school, and community.

In order to participate in these activities, the athlete and parent/guardian must read the terms provided below, acknowledging that they will abide by the rules and requirements of D50 athletic teams and clubs. An athlete and their family will only be required to complete this form once each year to cover all involved sports and clubs.
