LOUIS Oct 22 D-Cohort TSP Evaluation: Wrap Up Phase 1
This survey is structured in 2 parts. A more general program evaluation will be followed by more personal reflections on the progress of your projects and where you see yourself go in Phase 2.

Please note that this evaluation is intended to gather feedback on the Textbook Success Program Phase 1 only, not the broader grant or institutional structures that may be supporting your OER project work. Please keep your responses focused to the program, rather than grant logistics (we suggest you communicate any feedback about this directly to your institutional points of contact).

This survey should take you between 5-10 minutes to complete. Your responses will be anonymous and your feedback is used to:
1. Gather information on the value and impact of the program Phase 1,
2. Improve the program's efficacy — be it relating to the curriculum, structure, facilitation, or tools,
3. Inform your customized support during Phase 2.
