Assessment Opt-Out Form
Parents or guardians of CPS scholars may choose to keep their scholar from participating in some required district assessments and surveys. However, the decision to opt-out any scholar from district assessments and surveys must conform to the Board policy as outlined below.

In order to achieve the purposes of the student assessment program and comply with state and federal law, the district requires all enrolled scholars to participate in all applicable aspects of the district assessment program, including statewide assessments. Policy IL

All district assessments are listed on the District Assessment and Survey Calendar, located on the CPS website or available from your scholar's school. Some assessments are ineligible for opt-out. See complete list on the Assessment & Survey Opt-Out website.

This request must be completed for each scholar. Do not put multiple names in the blanks.
