2023-2024 Honors Program Recommender Request Form
Deadline: February 1, 2023

This form is to submit the names and email addresses of your recommenders. You may submit this form before submitting your Honors Scholarship Application. You may do this to allow your recommenders an adequate amount of time to complete recommendations on your behalf.

It is your responsibility to make sure they have received the form to complete on your behalf. Check with them after submitting this form to ensure they have received an email with the form to complete. It may take a couple of days, (especially if you complete this form over the weekend) for them to receive an email. It is not sent automatically, a staff member in the Scholarship Office will email the form to your recommenders.  We will begin sending these emails around the middle of September. 

Remember: your application is incomplete without THREE completed recommendation forms. Please follow up with your recommenders!
