Registration Form

Our programming for 1:1 services is on a quarterly basis (according to the yearly calendar). Our next quarter starts in July 2023. 

We will reach out to families on the waitlist on June 1st with a form for you to indicate interest and scheduling.

After you submit that form, we will contact you with proposed weekly time spots for continuing appointments. If we find something that works for you, we will complete intake paperwork and any evaluations that need to take place before the quarter starts in July.
