In-Person Enrollment Request: Hale Charter Academy 2020-21

If, for whatever reason, you are unable to complete the online Enrollment Packet and Verification Documents, we will schedule a time for you to pick up blank forms, and drop off completed ones at the school. Please complete the following form and we will contact you with a date and time.
Visitor Notice
Effective immediately, all visitors to the school must pass a quick screening by visitor personnel, affirming:
1. Have you been out of the country in the last 14 days?
2. Have you had contact with anyone confirmed to have COVID-19 in the last 14 days?
3. Have you had any of these symptoms in the last 14 days?
Fever greater than 100 degrees
Difficulty breathing
Cough
4.Have you taken medication to reduce fever in the last 14 days?
If the answer is yes to any of these questions, you cannot come to the campus.
When you are on campus you must wear a mask, and maintain a 6-foot separation from others at all times.
