PATHFINDER VIRTUAL CHOIR AND ORCHESTRA
Dear Parent or Legal Guardian,

Thank you for allowing your Pathfinder child to be a part of the Ontario Conference Pathfinder Virtual Choir and Orchestra. It is proposed that this video will be shown and released online on the occasion of the 70th Anniversary of Pathfinder Ministry on September 19, 2020.

By filling out this online form, you give permission to the Ontario Conference Pathfinder Ministry, and/or parties designated by the Ontario Conference Pathfinder Ministry to use your child's photograph(s)/video(s) in all forms of media, for any and all promotional purposes including advertising, display, audiovisual, exhibition or club ministry use.

Further, you understand and agree that you or your child will not receive any payment for the time or expenses or any royalty for the publication of the photograph(s)/video(s) or the use of your child's name and you hereby release the Ontario Conference Pathfinder Ministry, and/or parties designated by the Ontario Conference Pathfinder Ministry from any such claims.
