Date of Service *
Contact *
Your name: Person from the Sunday Service Committee who is coordinating with the speaker.
Worship Leader *
Person who will lead congregation through the order of service. (May not be doing all of the aspects, but introduces what is next and who is doing it, if required.)
Tech Host(s) *
Person or people who will take care of the tech for the service.
Speaker *
The person giving the lecture, talk, or sermon, or the person who is leading the activities (such as during flower communion or water in-gathering).
Title *
Usually the title of the lecture, talk, or sermon. But could be the title of the service as a whole. "Membership Sunday," for example.
Newsletter Tag *
One sentence that describes the service - for the front page of the print newsletter.
Blurb *
Longer description of the service - for the online calendar, news releases, etc. Should include biographical notes about the speaker.
Principle *
While many talks could fit under more than one principle, pick the one that is most relevant.
Speaker's Email Address *
Required so we can send the Zoom link to the speaker!
Speaker's Website
If the speaker has a website, list it to assist Pat in writing news releases.
Speaker's Mailing Address *
Required so we can send the speaker a check. (If this is a person listed in the church directory, you can can just write "on file."
Speaker Fee *
Required so the bookkeeper can write a check.
Notes
Anything else you want to note. For example, Forum follows the service, or this is start of National Dog Week, etc.

Clear form
Never submit passwords through Google Forms.
