Fill out this info for the person the scooter will be registered to. Typically that's the scooter driver but sometimes parents or guardians want the scooter titled in their name. If the registered owner is under the age of 18, there MUST be a parent or legal guardian co-owner registered to the vehicle as well.  Registrant must be present to sign all paperwork.

If you do not have a local Gainesville address or don't know it yet, leave this part blank. We take care of all the paperwork the tax collector needs to register and title your scooter. Your registration is what ties you as an owner to a vehicle with a specific tag number and is renewed every year through the State of Florida Tax Collector's Office.  Your title shows that you are the owner of the vehicle and you will need this if you ever want to sell your scooter.
