I understand that I must provide all of the supplies and equipment necessary to service my booth including water, tables,
and chairs. By signing this form, I agree to hold harmless the Vinita Area Chamber of Commerce, City of Vinita, or anyone
else connected with this event from any and all claims, demands, causes of action, liability damages, costs and expenses of
any kind for loss of life, personal or bodily injury, or damage to property incurred by myself, my organization, employees,
agents, or servants incurred before, during, or after the Annual Worlds Largest Calf Fry & BBQ Cook-Off. Property and
liability insurance is my responsibility. By submitting this application, participants agree to allow their likeness and
property to be used in subsequent publicity photographs and all media. The sales tax rate is 9.5% and must be collected
and paid on applicable sales either directly to the Oklahoma Tax Commission if you have a permit or to the Vinita Area
Chamber of Commerce if you do not. All registrations require payment in full at the time of registration. Registration fees
are nonrefundable.
