Covid Vaccine Request Form: There will be a vaccination clinic at Lindenwold High School for the Lindenwold community and students administered by the New Jersey Department of Health on Thursday, September 2, 2021 from 5:00 p.m. to 8:00 a.m. p.m. For children 12 to 17 years old, the vaccine is the Pfizer vaccine and a second injection will be scheduled at the time of the first injection. Adults 18 years of age and older will receive the Johnson & Johnson vaccine. This is only ONE injection and no follow-up appointment is needed.         --------------------------------------------------    Formulario de solicitud de vacuna de Covid: Habrá una clínica de vacunación en Lindenwold High School para la comunidad de Lindenwold y los estudiantes administrada por el Departamento de Salud de Nueva Jersey el jueves 2 de septiembre de 2021 de 5:00 pm a 8:00 pm. Para los niños de 12 a 17 años, la vacuna es la vacuna Pfizer y se programará una segunda inyección en el momento de la primera inyección.
