I understand that I am responsible for my own travel expenses in getting to Anaheim, and lodging while in California. The Conservatory is not to be involved with travel arrangements of any kind.

I would be interested in splitting the cost of an Air BNB or VRBO with other studio dancers/parents. If I mark yes on this question, I agree to have my contact information shared with others who marked yes on this question, so that we may plan things. The Conservatory will not be involved with any aspect of lodgings, other than facilitating this beginning communication. *
I understand that reservations for the performer and guest tickets are set and cannot be changed. I understand that if I buy guest tickets from another source, or if I am using a Magic Key, I must reserve the same parks and dates as those on the performer ticket.

There will be between 6-10 weeks of rehearsal for the parade - dependent on when the Conservatory receives the video of the Disneyland Parade choreography. My child will be expected to be at all parade rehearsals, which will be held on Monday Evenings or Saturday afternoons - depending on how many dancers register, and how many rehearsal groups we need to divide into. Final groups and schedules will be sent out the end of August, with rehearsals beginning the first or second week of September. Zoom may be an option for those dancers unable to attend rehearsals in person. *
I would like order additional Conservatory Disney Swag for myself or others in my travel group.   *
Installment payments will be due the 1st of each month. A late fee of $10 will be added if not paid by the 5th of each month. The studio accepts Venmo @ClassicalB , Cash, and Checks as forms of payment. If for any reason I need to drop out of the parade, I MUST give the Conservatory notice BEFORE the 15th of August. (In that event, all money expect the initial deposit will be refunded in full.) I understand that if I do not pay in full, my child will not receive their parade costume and will not be allowed to perform in the parade. *
I give permission to Classical Ballet Conservatory, its owners and operators to seek medical treatment for my child in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.As the legal parent or guardian, I release and hold harmless Classical Ballet Conservatory, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Classical Ballet Conservatory, its owners and operators or in route to or from any of said premises.                       Waiver/Release For Communicable Diseases Including COVID-19In consideration of being allowed to participate in Classes and related events and activities, the undersigned acknowledges, appreciates, and agrees that:    Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,    I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,    I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases.  Classical Ballet Conservatory  may require dancers, volunteers, parents, and/or audience members to wear masks and/or take other measures such as self-screening for COVID-19.  Dancers, volunteers, parents, and audience members agree to comply with such measures and understand that they will not be allowed to participate if they choose not to comply with such measures.  If I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,    I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Classical Ballet Conservatory , its officers, agents, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law. *
I hereby give permission for images of my child, captured during class or performance, through video, photo and digital camera, to be used solely for the purposes of Classical Ballet Conservatory and/or Epic Dane Tours promotional material and publications, and waive any rights of compensation or ownership thereto. I also understand that My child may have photos and/or video taken of them to be used for rehearsal purposes, and may be posted to the app, sent out through email, and or posted to social media. Children will NEVER be tagged or named on any social platform, unless consent is given by guardians, or unless the student/guardian tags themselves or the studio in an image - which is then giving the studio permission to repost. *
