Field Day T-Shirt Order Form 2023
Dear Parents,

The children will be participating in Field Day under the direction of our physical education teacher, Mrs. Sylvia Dodd.  Students in Grades K through 8 will showcase their athletic skills, teamwork, and their ability to have fun!

The t-shirts that the children wear on Field Day are designed by an 8th grader and the classes are coordinated by color for easy team identification.

Please indicate your child's name, class, and shirt size below. If you have more than one child, please submit this form for each child so we can make sure all children in corresponding grades will receive the size of their choice. Please note, that due to t-shirts being pre-ordered, we are unable to exchange sizes.

OPTIONAL: To add to the spirit, we are offering parents the opportunity to purchase a class t-shirt for themselves. The cost is $10 a shirt and must be ordered and paid for by May 5th. Checks should be made payable to St. Dominic PTA and sent in a clearly marked envelope to PTA Field Day T-Shirt Order with your family name and homeroom on it. PLEASE NOTE: If payment is not received then your parent shirt will not be ordered.
