Academic Improvement Plan (AIP)
This Academic Improvement Plan is a tool for students who are on Academic Probation. You must complete this Academic Improvement Plan if on Academic Probation.

This form is in two parts. The first ("Pt1") asks you to reflect independently on your academic progress ("Reflection," "Plans for Success," and "Availability"). Once you've completed the reflection portion, skip to the end of the form and click "submit." You will receive a copy of your responses, which you will need to forward to your advisor. Do not delete the email for you will need it to log back in and complete the second portion with your advisor.

When you meet with your advisor, you will log back into the form so that the two of you can complete the "Advisor Input" section together ("Pt2"). Forward your completed form to Jackie Molina (jmolina2@luc.edu). Please include your advisor on the email so that Jackie can confirm the two of you met.
