By selecting, "yes," I understand that my student is enrolling in the Arts Academy of the Fine Arts Department of Ben Lippen School for the Fall 2023 semester. I agree that my student will participate in fifteen, thirty-minute lessons between  August 21, 2023-December 4, 2023 *
By selecting, "yes," I give Ben Lippen School permission to charge a private lessons fee of $624 to my student's FACTS account. I agree to pay this amount in full. *
By selecting, "yes," I understand that make-up lessons will only be given in the event of an instructor absence or school closure. *
By selecting, "yes," I understand that private lessons are a full-semester commitment and that students who join late or drop out early will be billed for a full semester. *
By selecting, "yes," I understand that my student must have access to his or her private lesson instrument at home. I agree to provide this instrument and any music method resources recommended by the instructor. *
By selecting, "yes," I agree to give my student's private lesson instructor twenty-four-hour notice of a pre-arranged absence. *
