Please use this form to submit your uniform order.  All Items on this order form are ADULT SIZES.

You are required to purchase a JERSEY and RACING SHORTS OR 1/2 TIGHTS (if you already have a jersey and/or shorts you do NOT have to purchase another - they are the same as the XC uniforms as well).

Payment must be made by cash or check (payable to PCCS - Salem Track) ASAP to Coach Goodridge.  Please bring payment in an envelop with your name on it.

If you wish to order more than one of an item, you can submit multiple orders.

You are responsible for totaling your Order Form correctly!  Once the order has been placed you are responsible for payment.  Payment is due by Friday, March 19th.

All uniforms will arrive by our first meet of the season.

Remember the only color visible under the uniform is solid WHITE!

The FINAL DAY to place a uniform order is Friday, March 19th.
