Please complete the following survey to sign your child up for Kindergarten Round-Up. Once your information has been received and processed, a confirmation letter with your appointment time and physical form will be mailed to you.

Kindergarten Round-Up will be held on Thursday, April 4th from 8:30 a.m.-6:30pm.  Please select ONLY 1 date/time for your first preference and 1 second preference appointment times. You will need to bring your student, their original birth certificate, physical, immunization records, and custody paperwork (if applicable).  Students must be 5 by August 1st.
