Veterinarian Information

Please call your veterinarian's office and have them send us a current copy of all the following vaccinations, including a veterinarian signed rabies certificate.  We must have a copy of current vaccinations in our office by email or paper before any boarding reservations
will be made.  Email records to records@pethavenpets.com

- Pet Haven’s Client/Pet information form
- Current Rabies (Certificate must be signed by veterinarian)
- Current Distemper Parvo
- Current Bordetella (kennel cough)
- Current Negative Fecal Result (we require a new fecal result every 6 months and this
must be current during your boarding stay)

Available times for Pick-Up & Drop-Off are:
Monday 8 a.m. - 5 p.m.
Tuesday 8 a.m. OR 5 p.m.  We are not in the office on Tuesday outside these times.
Wednesday 8 a.m. - 5 p.m.
Thursday 8 a.m. - 5 p.m.
Friday 8 a.m. - 5 p.m.
Saturday 8 a.m. OR 5 p.m.  We are not in the office on Saturday outside these times.
Sunday drop-off is not available.
Sunday pick-up is available at 5 p.m. only for an extra fee.
