We are formulating a plan for an outdoor promotion ceremony. We are using our state's most recent guidelines as our framework for the celebration.

At this time, we are asking for you to complete the short survey below to make us aware if you plan on attending or NOT ATTENDING the in person ceremony. Your answer on this survey will allow us to work on the proper framework and guest number moving forward.
