Your survey responses will deepen our understanding of the individual and collective strengths and challenges of women veterans and will guide us in building more supportive communities together.


To be eligible for a $25 e-gift card, you MUST:

1. a) Identify as a woman, b) Identified as a woman during your military service, and c) Served in the U.S. Armed Forces, Reserves, or National Guard.

2. Be a current resident of Benton, Lincoln, or Linn County, Oregon and/or identify as Alaskan Native, American Indian, or Native American and be connected with Grand Ronde or Siletz.
 
3. Complete and submit the survey.

Your personal information will only be used by Red Feather Ranch for one or more of the following actions:

1. Email your e-gift card if you are eligible.
 
2. Schedule an interview with you for which you will receive a second $25 e-gift card upon completion.
 
3. Invite you to participate in future Red Feather Ranch events and activities. You may unsubscribe from these emails at anytime.  

Data from this survey, excluding any personal identifying information will be provided to our grantor: IHN-CCO (InterCommunity Health Network Coordinated Care Organization), Samaritan Health, and Oregon Health Authority.
