Parents with children wishing to attend Sunday's Well Boating & Tennis Club are required to complete this self-report screening form as a pre-condition to authorized participation in any junior activity.

- Please complete 1 form for each child.
- A new form will need to be completed every week (all forms expire on Saturday of the week they were completed).
- Please complete the form within 24 hours of first attending the club for that week.
- Please make sure to select all the activities your child will be attending in the current week up to and including Saturday (please only tick the activities you are certain about). If you decide at a later date to join an additional activity, then a second form should be completed no more than 24 hours before the start fo the activity.

NOTE: A child will not be admitted to the activity if a form has not been completed.
