I hereby give my child permission to attend the Lady Lions Softball Camp. I understand that this camp includes physical activity and that my child will attend camp in the proper attire that is safe for such activity including cleats and socks. I understand that softball is a physical activity where possible injuries may occur. By digitally signing my name I give the CHS Triple L Booster club the right to seek medical attention. *
