BtB Tutoring Program - 4th Quarter
Welcome to the BtB Crew! We are so glad you are here.

 Please complete this form to register your student for the BtB tutoring program for the 4th Quarter sessions. Tutoring sessions are every Tuesday and Thursday from 3:45-4:30pm. The sessions will run from Tuesday, March 15th through Thursday, May 12th.  You may choose from the following options: Tuesdays only ($112 for 8 sessions total), Thursdays only ($112 for 8 sessions total) or both Tuesdays and Thursdays ($224 for 16 total sessions). Payment is required for every session offered in the 4th quarter, regardless of attendance and credits will only be issued for excused absences. Registration is on a first-come, first-serve basis and spaces are limited.

Students are dismissed to the Community Room where we serve snacks and have some downtime before heading over to the High School Art Room in Big Ben. Tutoring begins promptly at 3:45pm and ends at 4:30pm. Should you need your child to stay with the BtB Crew after tutoring, we are here until 6pm.

The Franklin Beyond the Bell Program agrees to provide written notification of all changes in policies and two weeks’ notice for any change in fees.

I understand that my child will be released ONLY to those listed on my Emergency Contact/Authorized Pick Up List and agree to amend my list as needed. I also understand when my child has been released to one of the above named people, his or her welfare is no longer the responsibility of Franklin Beyond the Bell (BtB) *

The staff has permission to evaluate simple injuries and apply first aid if necessary. In the event of serious injury or illness and if I cannot be reached, my child has permission to be treated by his/her Physician, by the medical personnel if 911 is called or hospital emergency services. If 911 is called and transportation by ambulance is necessary, I give my permission for transport. *

I agree to pay the registration fee ($15/child or $25/family) during registration. I will reserve my student(s) BTB days and pay all fees by the Friday before the week my student(s) attend. I understand that prepayment for all BtB services, clubs, programs and events is required. The daily fee is $14, $25 for 1PM and 12 PM release days. $3 for before school care. *

I give permission for the Franklin Beyond the Bell Program to photograph my child while involved in after school activities. Photographs may be placed on our bulletin boards and website for parent and children viewing. I understand that my child’s identification (name and age) will not be disclosed. *
I understand that if my child does not attend a tutoring session they are signed up for and the absence was not excused then I am to pay for the session regardless. *
