Complete ONE form for EACH CHILD.  Form is required to be completed by the first day of school, August 22. 2023.  Includes Contact Information, Emergency Information Contacts, Transportation and Medical Information, Field Trip Permissions, PTO Directory Information and more.  All information will be kept on file for the current school year.  Any updates during the year should be sent to kmizrachi@torahacademy.org
