We, the parent(s)/guardian(s) of the
registrant, agree that the registrant and we will
abide by the rules and the Code of Conduct of Lenape Valley
Basketball Program. We give our approval
for the registrant's participation in
all tryout activities. We assume all risks and hazards
incidental to the conduct of the activities and recognize the possibility of injury to the
registrant.
NOTE: Completion of this form and participation in tryouts does not
guarantee the registrant a position on the team. Player
selection is a competitive process
and invitation to join the Lenape Valley Basketball Travel Team will
be made based upon skills
performance, as evaluated by the
coach and the evaluators. This
form is only
for the use of the Travel Basketball Coaches.

Basketball is a competitive program. We
do require the full cooperation and support
of all players and parents. We stress positive outlook and aim to create a positive attitude
with all of our teams.
