BACKGROUND
The  Kiwanis-Family  Relations  Award  is  designed  to  recognize  and  honor  those  CKI  clubs  that  have  participated  in  activities  and  events  to  enhance  Kiwanis-Family  relations.  

GENERAL GUIDELINES:

1. Read  the  entire  form  carefully  before  beginning  work  on  your  application.  

2. This  award  form  should  accompany  the  entry  essay  and  supplemental  materials.

3. You  must  review  the  award  submission  and  sign  the  signature  of  verification  form.  The  signature  is  only  attesting  to  the  accuracy  of  the  information  within  the  application.

4. This  award  must  be  submitted in this form  with  the  accompanying  signature  certification  sheet.  ALL  PARTS  MUST  BE  RECEIVED  BY  January  31st,  2020.  There  will  be  NO  EXCEPTIONS.

5. NOTE:  All  Documents/Awards  emailed (if not submitted through this application)  to  the  Awards  Chair  account  (LRA@cdcki.org)  must  be  contained  in  ONE  email.  

6. The  activity  described  shall  cover  any  program  of  service  performed  between  the  2018  District  Convention  and  the  2019  District  Convention.

RULES  GOVERNING  THE  OUTSTANDING  KIWANIS-FAMILY  RELATIONS  AWARD

1. To  be  eligible  for  this  contest,  a  club  must  be  in  good  standing  with  both  the  District  and  International.  


4. The  time  frame  of  this  award  is  from  the  2019 District  Convention  to  the  2020 District  Convention.
 
5. Certification,  by  signature,  of  the  accuracy  of  the  information  provided,  must  accompany  the  entry.
 
6. Decisions  of  the  judges  are  final.
