Purpose of the research: Odontophobia, which is the fear and anxiety individuals experience when receiving dental treatments results in poor patient adherence and inadequate oral health. This study will investigate various factors and their influence on odontophobia.

What you will do in this experiment: You will take a survey consisting of various questions about your experiences with dental treatments and dentists. You will also be asked some demographic questions.

Time required: 15-20 minutes

Risks: If you have had previous traumatic experiences pertaining to dentistry and/or dentists, you may feel distraught when reading questions that remind you of them. You are allowed to skip any question you find disturbing and may log off at any time from the study.

Benefits: Upon completion of this study, a thorough explanation of the experiment and the hypotheses will be provided. You will also be provided with information of possible ways to minimize dental anxiety.

Confidentiality: Your participation in this research will remain confidential and your responses will be shared only among those researchers associated with this study.

Participation and withdrawal: Your participation in this study is voluntary.  If you feel uncomfortable answering any questions during the course of this study, you may skip them.  Also, you may withdraw from the study at any time without penalty.
