Complete this form if your student is planning to take a virtual course as one of his/her 7 class periods/credits during the 24-25 school year. You do not need to complete this form if your student is taking a virtual course in addition to his/her 7 class periods next year.

Parent/Student is responsible for transportation if choosing “Off-Campus" virtual. Students can only have one "On-Campus" lab period per school year.
