Welcome to our re-enrollment form for current Pre-K families. Please fill out this form and submit your answers at the end. If you are a new family to St. Mary Catholic School, do not fill out this form, fill out the New Family form.
Students must be 3 years old and potty-trained prior to beginning classes for PK3.
Students must be 4 years old and potty-trained prior to beginning classes for PK4.
Students must be 4 years old and potty-trained prior to beginning classes for All Day PK4 (formerly TK).
Up-to-date immunization records on the Washington Department of Health Certificate of Immunization Status (CIS) form and copy of birth certificate are REQUIRED by child's first day of school.
PK3 & PK4 begins at 8:00am and ends at 11:00am. All day PK4 begins at 8:00am and ends at 2:45pm, offering a longer learning day. There is no nap time for all day PK4.
Students are expected to be on time consistently (arrive by 8:00am, leave at 11:00am/2:45pm for All day PK4 -or continue to Educare after class ends) and attend consistently on their scheduled days. Excessive tardiness or absenteeism is hindering to the learning process and disruptive to the other students and the classroom routine. By enrolling your child(ren) in our Pre-K program you are committing to make every effort to make sure your student(s) are on time to class and attend regularly.
Children cannot attend Educare on days they did not attend Pre-k class with the exception of the occasional appointment for doctor, dentist, etc.Educare begins at 11:00 and closes at 5:30pm on normal school days.
