School Based Health Center Survey
School-Based Health Centers (SBHC) provide basic physical, mental, and have the potential to provide other health services as needed. The health center provides more services than a school nurse and is not to replace the school nurse. The goal is to provide a resource center for wellness and prevention in an effort to reduce barriers to learning, such as absenteeism, mental health issues, and trauma. It is our hope that by having a school-based health center in our district we can promote health and educational equity by: • Increasing school attendance • Supporting at-risk students • Connecting students and families to health coverage • Expanding capacity through partnerships in the community • Promoting health education, nutrition, and physical activity • Providing reimbursable health services in the schools • Advancing preventive health care • Managing chronic disease • Establishing a culture of wellness and healthy habits • Reducing disciplinary referrals • Increasing graduation rates. We need your input. Please fill out the survey below to the best of your ability. There are only 10 questions and the survey should take less than 10 minutes to complete.
