KVKL Incident Report Form
This form is to be used to report any conduct violations which a member of the League would like to report to the KVKL Board of Directors for action or awareness.

Please note that upon submission, your form responses will be sent to the nine members of the Board, who will keep this information confidential unless and until you provide permission otherwise. If you provide your contact information and permission for the Board to follow up with you, someone from the team will reach out to you within 3 days.
