Ostomy Supply Application

The Ostomy Support Group of North San Diego County (OSGNSDC) is committed to helping uninsured San Diego area residents secure ostomy supplies through our Ostomy Supply Closet. Our Supply Closet is run by non-medical community volunteers. All the ostomy supplies in this closet have been donated. Therefore, we may not have the exact brand requested.  Fortunately, we stock several high-quality brands that may accommodate your needs.

  • After completing the application below, it takes 5-7 days for our volunteers to process your request. Why do I need to complete a form? Ostomy supplies vary greatly and one size does NOT fit all. We want to make sure you receive supplies you can actually use!
  • We are unable to accommodate emergencies, as we are non-medical community volunteers and do not have a store front, office, staff or employees.

  • When and Where to pick up supplies: 12 pm -2 pm on Feb. 24,  Mar. 30,  Apr. 27,  May 18, No June, July 27, Aug. 24,  Sept. 28,  Oct. 26,  Nov. 23. at Cube Smart storage (gated) 1625 W. Vista Way. Vista, CA. Text or call 1-760-271-5046 for reservations, questions and gate access

  • For the safety of our volunteers, we do not deliver ostomy supplies.
  • You may request 30-days worth of ostomy supplies every 30 days.
  • A $25 donation is suggested. These donations are used to cover storage facility costs for our Supply Closet
