This form is for anyone within the State of Nevada that has submitted a medical and/or religious exemption request regarding receiving a COVID-19 Vaccine, and that was TERMINATED from their employment despite submitting an exemption.  

AT THIS TIME, WE ARE ONLY SEEKING RESPONSES FROM INDIVIDUALS WHO HAVE ACTUALLY BEEN TERMINATED FROM THEIR EMPLOYMENT.  

Filling out this form will assist Jennings & Fulton, LTD in determining whether individuals may have a claim for wrongful termination that can be filed with the Equal Employment Opportunity Commission or a state/federal court case.
