To direct Company Matching Donations to UMS Music Department ASB Account:

IMPORTANT : Do not process a matching donation if your employer requires your contribution be submitted via their matching donation portal. The funds will not always arrive on time and your contribution will not be applied to your Student CCD or your Disneyland Trip account. 

Please make your donation via check or on our UMS School Webstore and request a receipt to submit to your employer.

The company portion (match) of this donation will be applied to the overall Music Department or Trip to benefit all students attending.

I understand CCD, Trip & Instrument Maintenance, is a separate donation from any made to UMS Home and School Club (HSC) or UMS Music Boosters. 

HSC matched portion of donations are not applied or transferable to Music Department CCD or Disneyland Personal Trip Accounts. Employee portion of  Contributions for the TRIP, CCD or CLUBS should not be made processed through your company matching donation portals.  HSC has a different Tax ID

Send a check payable to : Union Middle School for the amount and request a receipt to submit to your company for any matching donations.
