Certificate of Completion Request 
for Continuing Education Programs

Use this form to request a certificate of completion for the program that has been finished. All modules must be completed with a P grade and all invoices must be paid before a certificate will be issued.  If you have any questions, contact the Continue Education department at continuingeducation@pcu-whs.ca. First certificate is free, additional copies will be $15 each.
