In order for your child to transport his/her school issued device between home and school, the following form must be completed for each of your students.

We have established the following guidelines for 2022 - 2023 school-issued devices. By checking the box below you acknowledge that you understand the guidelines. You will be financially responsible for the cost to repair and/or replace the device and charger if it is: (1) not returned (2) intentionally damaged (3) lost or damaged because of negligence (4) stolen, but not report to school and/or police by the end of the next school day.
